Initial / Renewal Badge Application:
To be used for new badges and badge renewals for all employees of tenants, airlines, vendors and construction companies. Must be filled out completely, including Section 2, Company Information/Authorizing Agent Certification. A warrants check will be conducted, as well as a Driver’s License records check for those individuals needing on-airport driving privileges. You must also provide two (2) forms of identification (refer to the List of Acceptable Documents). PLEASE DO NOT USE TO REPLACE LOST OR STOLEN BADGES.

Badge Status Change Application: To be used to replace lost and stolen badges, as well as adding or removing designations to an applicant's badge.

Bike Path Badge Application:
Badge application form to apply for a card that will open the gates for the south perimeter path (bike path) located at the Salt Lake International Airport. This form needs to be completed if you do not already have an airport-issued identification badge.

Ground Transportation Driver Badge Application:
Initial badge application for ground transportation drivers.

Ground Transportation Driver Renewal Application:
This application must be filled out completely. A warrants and Drivers License records check will be conducted. You must also provide two (2) forms of identification (refer to the List of Acceptable Documents), and a valid Driver’s License. Complete this form for renewal, replacement of a lost/stolen or expired badge, or to request a badge status or name change.

Ground Transportation Starter/Loader Badge Application:
Initial badge application for individuals working as a starter/loader at the Salt Lake City International Airport.

SLCIA General Aviation Badge Application:
Initial badge application for individuals working in general aviation areas at the Salt Lake City International Airport.

SLCIA Hangar Card Application:
Initial badge application for hangar tenants at the Salt Lake City International Airport.

South Valley Regional Hangar Card Application:
Initial badge application for hangar tenants at South Valley Regional Airport.

Temporary Badge Application:
Use this form for individuals who will work at the airport on a temporary basis (not to exceed seven days) and will not be issued a permanent ID badge.

Ramp Permit Application:
Use this form for requesting a ramp permit

Parental Consent:
This form must be completed and notarized to allow an applicant under the age of 18 to receive a badge.
